Bluetooth: HFP_HF: Optimize SLC init procedure

In current implementation, it is difficult to insert new AT commands
in the SLC initialization sequence.

Add a SLC initialization sequence. Send AT command one by one.

It will be easy to insert new AT commands in the appropriate position.
