bluetooth: doc: Reformat BT_APPEARANCE_* defines

Split the BT_APPEARANCE_* doc on two lines as several lines were
dangerously close to 100-character line limit + some constants were
misaligned.
This makes follow-up commit possible without breaking code formatting
rules.
