Bluetooth: Host: New API: bt_get_appearance()

The this change is a step towards make the GAP appearance runtime
settable and ATT writable.

The new function bt_get_appearance() wraps CONFIG_BT_DEVICE_APPEARANCE
and should replace all its uses in code.
