Bluetooth: Audio: Make CAP acceptor more explicit

Remove the generic CONFIG_BT_CAP and CONFIG_BT_DEBUG_CAP
and only use Kconfigs specific to the individual roles.

This also includes renaming cap.c to cap_acceptor.c.
