debug: remove DEBUG_INFO from EXCEPTION_STACK_TRACE

The `DEBUG_INFO` in the `EXCEPTION_STACK_TRACE` is only
required by x86. Move that to `X86_EXCEPTION_STACK_TRACE`
instead.
