docs: networking: device drivers: convert dec/de4x5.txt to ReST



- add SPDX header;
- add a document title;
- mark code blocks and literals as such;
- adjust identation, whitespaces and blank lines where needed;
- add to networking/index.rst.

    Originally,   this  driver  was    written  for the  Digital   Equipment
    Corporation series of EtherWORKS Ethernet cards:

        DE425 TP/COAX EISA
