usb: Add Bluetooth device class core functionality

Implement Bluetooth over USB functionality through Bluetooth raw
access to the Bluetooth controller. Most devices with Bluetooth and
USB controllers supported by Zephyr can export themselves as USB
Bluetooth dongles.
